Medical Chart Bloopers


Real sentences taken from patients' hospital charts:

  • Patient lives at home with his mother, father, sister and pet turtle who is enrolled in daycare three days a week.
  • On the second day the knee was better, and by the third day it had completely disappeared.
  • Discharge status: Alive, but without permission.
  • Patient is numb from the toes down.
  • Occasional, constant, infrequent headaches.
  • Patient was alert and unresponsive.
  • Skin: somewhat pale, but present.
  • Patient has two teenage children, but no other abnormalities.
  • Patient has been depressed since she began seeing me in 1993.
  • While in ER, she was examined, x-rated and discharged.

LOL! I Love the Patient is numb from the toes down! LOL. All are too funny. No wonder people are mistreated all the time! Geesh!

Hey, I'm going to take a picture of a flower arrangement I made and try to email you. I've started trying to do the grave pieces myself. I mean, I KNOW that Brent's not REALLY there (he's in HEAVEN, of course), but it somehow makes me feel as though I'm doing something for him. Is that COMPLETELY crazy??? (I went to Hobby Lobby yesterday and they had ALL their flowers and grassy type stems HALF OFF!) Money was never really the issue, I guess it's like I said, just makes me happy to make something for him. ☺

Okay, sweet Friend, I'm off for a bit. I just wanted to hop over and say hello!

Ouch! As an ex-medical professional I cringe to think of the mistakes I made charting. These were funny! And I can see them happening! LOL!
